Dear Convenors, 

In order to ensure communication between HKHAMS and team captains are done in timely fashion, convenors should provide contact details for all team captains to HKHAMS. Please note that clubs should still communicate to HKHAMS through convenors. 
 
For any updates or changes of team captains/ representatives (one for each team) details for 2019-2020 season, we should be grateful if club convenors would submit their (i) names, (ii) ID card # (iii) contact numbers and (iv) email addresses to the ambrosekwan@hockey.org.hk & jowiewong@hockey.org.hk , on or before August 30, 2019 (Friday).  

The collection of this personal data will be used for the purposes of communicating information relevant to HKHAMS matters and also used for creating default login account names and passwords for the E-match card system. The person inputting the E-match card for your teams do not necessary be the team captains as they may not be present for all matches, you should share the login information to other team members who can back up when the team captains or representatives are absent at the match.
